Must have the education, training, and/or experience necessary to work in a rehabilitation setting, with beneficiaries. Maintain electronic beneficiary file on each beneficiary with timely notes with at least a bi-weekly update. Facilitate process groups as assigned. Make classroom presentations on program materials. Perform one-on-one counseling with an emphasis on spiritual regeneration and problem resolution with beneficiaries using scripture, personal experience and formal educational learning. Evaluate and present beneficiaries to Program Director for the case review team. Prepare an aftercare treatment plan and discharge summary for each beneficiary who completes the Program. Prepare and submit to the Program Director evaluation and written reports as requested, including but not limited to three-month reviews, five-month reviews and discharge summaries.
