Position Title: Bilingual Community Health Advocate

Job Description:

The Bilingual Community Health Advocate plays a crucial role in enhancing the well-being of patients and the wider community. As a vital connection between healthcare providers and clients, you will help remove barriers to accessing essential health and social services.

This position is primarily field-based, requiring travel to client sites and occasional attendance at the office for meetings or training. Candidates must be in reasonable commuting distance from the job location, in compliance with our hybrid work policy.

Key Responsibilities:

• Meet patients to identify social determinants affecting their health and well-being.
• Collaborate with the care team to create actionable plans tailored to patient needs.
• Build trust and provide advocacy to empower patients in achieving health objectives.
• Assist patients in navigating health-related services and community resources, including accompanying them to appointments.
• Facilitate clear communication between patients, families, and organizations to ensure cohesive support.
• Document interactions and patient progress accurately in medical records.
• Participate in team meetings as necessary.
• Travel to various work sites and other locations as required.

Minimum Qualifications:

• High school diploma or equivalent; experience with electronic medical records; at least one year in healthcare, community-based, or social work; or a combination of education and experience ensuring equivalent expertise.
• Bilingual proficiency in English and Spanish is essential.

Preferred Attributes:

• Certification as a Community Health Worker is a plus.
• Experience addressing challenges related to homelessness and employment is advantageous.
• Access to reliable personal transportation is preferred.

Salary and Benefits:
